Polk County Online Health Reporting


How do I use the Polk County Health Reporting System?

Each school day, navigate to https://healthreporting.polkcountyiowa.gov/ to access the login screen.

  1. At the login screen, enter your school's personal identification number (or PIN) and respective password. Sign in by clicking the "login" button. Your schools dashboard page will open. The dashboard page shows your schools information for a month history as well as data for the current school year.
  2. To enter the days data, on the top menu, click "Data Entry".
  3. Selected the date you wish to enter data for.
  4. Enter the number of students absent for health reasons.
  5. If the total number of students is not correct, enter the current number of enrolled students.
  6. Enter the number of staff absent for health reasons.
  7. If the total number of staff is not correct, enter the current number of staff.
  8. Please note of any other questions and enter data accordingly.
  9. The symptoms text-area is an input for you to put common symptoms that you have noted.
  10. Feel free to add any additional notes or comments to the notes section.
  11. Click the submit button to save the current entry.


What should I do if our absence rate is 10% or higher for students and/or staff?

Call the Polk County Health Department at 286-3890 and talk with a nurse on the Communicable Disease Investigation Team. A public health nurse will ask a few questions to identify a potential disease outbreak and provide health recommendations and/or guidance. If a disease outbreak is detected, then further assistance will be provided to your school by the Polk County Health Department. You may also call and talk to a public health nurse at any other time if you have health questions or concerns.
